The Pre-Kidney Transplant Advanced Practice Provider (APP) provides comprehensive evaluation, risk assessment, and care coordination for patients referred to for kidney transplantation within an Albany Medical Center transplant program. The APP collaborates with transplant surgeons, nephrologists, and the multidisciplinary team to ensure timely listing, regulatory compliance, and optimal candidate preparation.

Salary Range: $124,693-$145, 475

Essential Duties and Responsibilities

• •Conduct comprehensive pre-transplant evaluations, including history, physical examination, and risk stratification.
• •Review and interpret diagnostic testing (laboratory studies, imaging, cardiac and malignancy screening).
• •Identify and manage modifiable medical risk factors in collaboration with referring providers.
• •Prepare and present cases at multidisciplinary transplant selection meetings.
• •Ensure compliance with Organ Procurement and Transplantation Network (OPTN) and United Network for Organ Sharing (UNOS) requirements.
• •Facilitate timely listing, waitlist maintenance, and documentation accuracy.
• •Provide patient and family education regarding transplant processes, risks, benefits, and immunosuppressive therapy expectations.
• •Coordinate recipient readiness for deceased and living donor transplantation.
• •Participate in quality improvement initiatives and regulatory audits.
• •Contribute to academic activities including education of trainees and protocol development as appropriate.

Qualifications, Education, Licensure, and Certification Required:

• •Graduate of accredited NP or PA program.
• •Current state licensure and national board certification.
• •Experience in transplant, nephrology, or surgical specialty preferred.
• •Demonstrated knowledge of transplant regulatory standards.
• •Strong communication and interdisciplinary collaboration skills.
