What You’ll Need

• •A bachelor’s degree in human services .
• •A valid driver’s license .
• •A willingness to learn—we’ll provide all the training you need!

A Day in the Life

As an Aftercare Case Manager, you’ll support children and families who have achieved permanency through reintegration, adoption, custodianship/guardianship, or aging out of care. You’ll:

• •Focus on risk management and help families maintain stability during the aftercare period.
• •Provide crisis intervention and work with public and private agencies offering supportive services.
• •Maintain accurate documentation and reports.
• •Coordinate community and educational services for families.
• •Meet with families in-person to support reunification and resolve any concerns.
