Behavioral Health Case Manager

Part-Time : Approximately 20 hours per week

Hybrid : scheduled onsite days each month in Engelhard and Manteo

Pay : $23 - $27 per hour

Ocracoke Health Center, Inc. is currently seeking a Behavioral Health Case Manager to join our Integrated Care Team. This position will work closely with primary medical providers and our Behavioral Health Counselor to provide case management related to referrals, linking patients to care, and addressing social determinants of health.

The position is hybrid and part-time, working approximately 20 hours per week. We are flexible in establishing the onsite schedule and will work with the selected candidate based on their availability and patient needs.

What You’ll Do:

• •Receive and manage referrals from primary care providers and behavioral health counselors.
• •Coordinate care and provide case management services, including assessment, care planning, follow-up, advocacy, and resource coordination.
• •Connect patients with specialty care, behavioral health services, and community resources that support their health and well-being.
• •Assist patients in addressing social determinants of health, including barriers related to housing, transportation, food access, financial stability, and other community support needs.
• •Collaborate closely with medical providers, behavioral health counselors, and other members of the care team to support patient goals and improve outcomes.
• •Provide administrative and care coordination support to the Behavioral Health Counselor and Integrated Care Team.
• •Maintain accurate, timely, and compliant documentation in the electronic health record (we use Athena).
• •Participate in team meetings, case reviews, and care planning activities.
• •Deliver patient-centered, culturally responsive services that respect the unique needs and circumstances of each individual.

Requirements

• •Bachelor's degree in social work or a related human services field required.
• •Candidates with an associate degree or actively pursuing a bachelor's degree and expected to graduate within one year may be considered.
• •One or more years of experience in case management preferred, particularly in a community-based or healthcare setting.
• •Strong verbal and written communication skills.
• •Experience with and/or ability to learn and use electronic health record systems (Athena experience is a plus).
• •Commitment to maintaining patient confidentiality and professionalism.
• •Knowledge of community resources and human service systems across the lifespan.
• •Ability to work collaboratively with patients, healthcare providers, community partners, and other members of the care team.
