## Mein Beruf Mensch.

  • Planung, Organisation und Evaluation von Betreuungsangeboten
  • Erstellung von Betreuungsplänen, Übersichten und Dokumentation sowie Kontrolle der Umsetzung
  • Zusammenarbeit mit Ärzten, Therapeuten, Pflege- und Betreuungskräften
  • Aktivierung und Durchführung von fähigkeitsbezogenen Angeboten der Beschäftigung
  • Dokumentation der durchgeführten Maßnahmen und Evaluationsergebnisse sowie besondere Vorkommnisse
  • Beschaffung von Materialien in Absprache mit der Einrichtungsleitung

## Ihr Profil.

  • eine abgeschlossene Weiterbildung nach § 43b, 53c SGB XI als zusätzliche Betreuungskraft (w/m/d) ist Voraussetzung
  • persönliche Eignung und Interesse
  • Soziale Kompetenz, Zuverlässigkeit, Kommunikationsfähigkeit, Durchsetzungsvermögen
  • Aufgeschlossenheit gegenüber dem kirchlich diakonischen Auftrag
