- General Responsibilities
1. Provide intake, screening, assessment, case planning, crisis intervention, and case management services.
2. Determine eligibility for Illinois Department on Aging Waiver Program (Community Care Program).
3. Conduct comprehensive needs assessments and determine eligibility for services.
4. Establish written care or treatment plans with clients and families for maximum independence.
5. Active intervention and advocacy to access necessary services from various programs.
6. Complete monitoring, follow-up, and proper termination of cases.
7. Ensure the completion of Intensive Casework with each completed comprehensive assessment.
8. Complete critical incident reports in accordance with Illinois Department on Aging Standards.
9. Maintain confidential case records and meet documentation standards.
10. Accurate completion of all required reporting and documentation.
11. Maintain cooperative relationships with service providers and community organizations.
12. Conduct educational presentations for professional and community groups.
13. Attend training, staff, and team meetings, and act as an Agency ambassador in the community.
14. Other duties as assigned within the guidelines of this position.

Requirements
- Bachelor's degree in social work, psychology, or related field from a regionally accredited institution. In some cases, a waiver may be granted by the Illinois Department on Aging if employee has significant case management or significant experience in aging services.
- Minimum of three months of experience in a related field.
- Preferred experience with program-specific populations.
- Fundamental communication and problem-solving skills.
- Ability to work independently and respond appropriately in crises.
- Relate well to individuals with disabilities and older adults, respecting their privacy and dignity.
- Effective and constructive interaction with diverse cultures and backgrounds.
- Proficiency in Microsoft Office applications (Outlook, Word, Teams).
- Strong verbal and written communication skills.
- Valid driver's license, reliable transportation, and proof of liability insurance.
