As the Ridge Case Manager , you will collaborate with other members of the clinical team, to serve as a liaison for the treatment team, client, families and aftercare services.

Key Responsibilities:

Provide comprehensive case management, including assessment, care planning, and ongoing support.

Participate in the interdisciplinary treatment planning process and perform proactive discharge planning.

Provide education to clients and their families regarding community resources, benefits, and coverage.

Coordinate and facilitate meetings between the clients/families/residential care team. You will assist clients and families in making difficult decisions while honoring their preferences and values.

Facilitate group sessions and support recovery-focused programming.

Ability to successfully document all clinical contacts and documentation through Electronic Medical System.

Support a safe, structured, and trauma-informed residential environment.

Facilitate and provide timely documentation.

Adhering to company policies and procedures regarding confidentiality and privacy.

Other duties may be assigned.

Qualifications:

Bachelor's degree in social work, psychology, healthcare administration, or a related field (preferred).

Minimum 2 years of experience in a residential or therapeutic setting.

Leadership skills that foster a culture of inclusivity and excellence.

Excellent communication, leadership, and organizational skills.

Strong crisis management and conflict resolution skills.

Salary Range

$28 — $30 USD
