Hiring Range:

$18.24 - $32.82 (based on a full time schedule of 40 hours per week).

Please note that the final offer will vary based on scheduled hours. Compensation for part-time or reduced schedules will be prorated based on scheduled weekly hours, experience, skills, qualifications, and internal equity considerations.

Provides culturally-sensitive health education, information, referrals and/or outreach to patients and their families and advocates for individual and community needs to improve health outcomes. Facilitates access to resources and/or provides direct services, such as informal counseling, social support, care coordination and health assessments.

I. Major Responsibilities:

  • 1. Acts as a liaison between the patients/families, clinical practice and community agencies and referral services. Remains a partner and source of contact for a family system until it connects with the appropriate services or until the family declines follow-up.
  • 2. Under general supervision, provides education, resources and/or referrals to patients and families.
  • 3. Advocates for patients and families in the clinic and community. Works in partnership with other service providers in the central Massachusetts community. Establishes and maintains effective working relationships with others including but not limited to staff, administrators, partnering agencies, elected officials, government agencies, businesses, customers, and the public.
  • 4. Meets regularly with the care team to review cases, identify next steps and priorities, and plan feedback to providers and community partners.
  • 5. Provides referrals and connects patients/families to community resources to help them overcome social problems and barriers.
  • 6. Demonstrates respect for individuals and groups with varied cultural, racial, ethnic, religious, and linguistic identities or backgrounds. Demonstrates understanding of human rights and principles of social justice.
  • 7. Plans and organizes work, preparing adequate records and reports, setting priorities, and maintaining a caseload in an effective and timely manner. Documents according to clinic policy and project requirements. Maintains records, reports, and statistics as required.

II. Position Qualifications:

License/Certification/Education:

  • Required:
  • 1. High school diploma or equivalent.
  • 2. Reliable transportation with a valid MA Driver’s license.
  • Preferred:
  • 1. CHW certification or coursework.

Experience/Skills:

  • Required:
  • 1. Experience working with diverse populations.
  • 2. Familiarity with local community.
  • 3. Excellent oral and written communication skills.
  • 4. Ability to work independently, and to function as part of a larger team.
  • 5. Understanding of how language, culture and socioeconomic circumstances affect health.
  • 6. Desire to work with diverse, multi-cultural and multi-lingual populations.
  • 7. Proficiency with Microsoft Office applications (i.e. MS Word, Excel, Access, Outlook) and web browsers.
  • 8. Proficiency with data entry and data tracking.
  • 9. Familiarity or experience with health promotion, health education, and/or healthcare terminology
  • Preferred:
  • 1. Fluency in one of the languages most spoken by UMMH patients (Spanish, Portuguese, Haitian Creole, Albanian, Arabic, and Vietnamese)
  • 2. One year of experience in a clinical office or community outreach setting.

Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.

Department-specific competencies, including age-specific competencies and their measurements, will be developed and maintained in the individual departments. The competencies will be maintained and attached to the departmental job description. Responsible managers will review competencies with position incumbents.

Position will be located at Blackstone Valley Technical School. Hours will follow school schedule.

The Community Health Worker will be a resource to students and assist in care coordination for health related social needs.
