About AbriCare

The Role

You'll be AbriCare's face across North Kansas City and own branch census growth end to end: market analysis, referral relationships, conversion, and billable hours across Medicaid and private pay. Community presence is the starting point; turning it into referrals, starts of care, and growing hours is the job. Everything you do must be ethical and defensible.

What You'll Do

Own the territory. Analyze the market, build a territory plan, and bring recommendations forward.

Build referral partnerships with hospitals, facilities, physician offices, AAAs, social service agencies, and community organizations.

Convert referrals into care, guiding partners and families through the Missouri Medicaid (DSDS) process and working with the branch to shorten time to start.

Represent AbriCare at senior events, health fairs, and partner offices, and present confidently to groups and families.

Run on data. Log activity in the CRM the same day and use it to explain results and adjust.

Stay compliant with the Anti-Kickback Statute, Stark Law where applicable, HIPAA, and Medicaid freedom-of-choice protections.

Success is measured by: referral source engagement, new referrals, referral-to-start conversion, time to start of care, billable hours added, net census growth, payer mix, CRM data quality, and compliance.

What You Bring

2+ years in healthcare business development, referral development, community outreach, or liaison work, ideally in home care, home health, hospice, or senior services.

A documented record of growing referrals, census, or pipeline against targets. Bring your numbers.

Strong presentation skills and comfort with Microsoft Office and a CRM.

High school diploma or GED.

Valid driver's license, reliable transportation, auto insurance and driving record meeting AbriCare policy, and willingness to travel daily.

Successful background screening per Missouri law and AbriCare policy.

Nice to have: a related degree; knowledge of Missouri Medicaid PCS and the DSDS process; established Kansas City aging-network relationships; experience launching a territory; bilingual ability.

Compensation & Details

Base salary of $65,000–$80,000 , based on experience
