Overview
- Provide direct care to and supervision of individuals served, ensuring their health, safety, and well-being.
- Implement Individual Service or Treatment Plans (ISPs/ITPs), Behavior Support Plans (BSPs), and program routines.
- Assist individuals with activities of daily living including personal care, meals, mobility, and household tasks.
- Facilitate activities that promote skill development, independence, recreation, and community engagement.
- Monitor and document progress, behaviors, health concerns, and incidents in accordance with policy.
- Communicate effectively with team members, supervisors, and other stakeholders to support coordinated care.
- Safely transport individuals to appointments, activities, or community outings as required.
- Maintain a clean, safe, and supportive environment while following all safety and regulatory requirements.
