## WIR SUCHEN:

Eine FACHKRAFT AMBULANT BETREUTES WOHNEN (m|w|d) in der Eingliederungshilfe für die Betreuung von erwachsenen Menschen mit psychischen Erkrankungen, Suchtmittelabhängigkeiten, sowie Menschen mit geistiger Behinderung 30-39 Std./Woche (flexible Arbeitszeit, nachmittags und teilw. am Wochenende)

## Ihre Aufgaben:

  • ➢ Übernahme von Fallverantwortung
  • ➢ Beratung, Begleitung und Unterstützung im eigenen Wohnumfeld sowie im individuellen Sozialraum
  • ➢ Förderung der Selbständigkeit unserer Betreuten und gemeinsame Entwicklung von realistischen Zukunftsperspektiven sowie einer individuellen Tagesstruktur
  • ➢ Unterstützung unserer Betreuten bei der Teilhabe am Arbeitsleben, in der Freizeitgestaltung sowie beim Leben in der Gesellschaft
  • ➢ Erstellung individueller Hilfeplanung
  • ➢ Kooperation mit dem Netzwerk der Betreuten
  • ➢ Krisenintervention
  • ➢ Dokumentation und Berichterstattung
  • ➢ Regelmäßige Teilnahme an Teamsitzungen und Supervision

## Ihre Qualifikation:

➢ Abgeschlossenes Studium der Sozialen Arbeit/Sozialpädagogik, sowie die benannten Berufsabschlüsse und Studiengänge unter Qualifikation Anerkennung Fachkraft auf unserer Homepage www.komm-mit-bewo.de

➢ Selbständiges Arbeiten

➢ Teamfähigkeit

➢ Fähigkeit zur Eigenorganisation

➢ Ausgeprägte Kommunikationsfähigkeit
