Für unseren Kunden, eine Senioren- und Pflegeeinrichtung in München, suchen wir im Rahmen der Personalvermittlung ab sofort Verstärkung als Fachkraft Pflegeüberleitung (gn) in Vollzeit. Sie werden direkt bei unserem Kunden fest angestellt.

  • Verdienst: Vergütung nach Tarif (AVR Caritas) zzgl. Zulagen
  • Start: ab sofort
  • Anstellung: direkt beim Kunden (Personalvermittlung)

### Ihre Aufgaben

  • Durchführung von strukturierten Beratungsgesprächen
  • Erstellung von ressourcenorientierter Pflegeanamnese und Ersteinschätzung des zu erwartenden Versorgungsbedarfs
  • Beratung bei der Gestaltung des Bewohnerumfeldes und Unterstützung bei der Organisation von notwendigen Hilfsmitteln
  • Planung und Koordination der Pflegeüberleitungen bei Einzug in die Einrichtung

### Ihr Profil

  • Eine aufgeschlossene, flexible Persönlichkeit, die gern mit und für ältere Menschen arbeitet
  • Selbstständige Arbeitsweise, Verantwortungsbewusstsein, Engagement und Teamgeist
  • Examinierte Pflegefachkraft mit mind. dreijähriger Berufserfahrung
  • Weiterbildung zur Pflegeüberleitung wünschenswert
  • Führerschein notwendig
