Multiplikator Geronto m,w,d gesucht

Wir suchen zum nächstmöglichen Zeitpunkt zur Verstärkung unseres Teams eine Multiplikatorin Geronto (m/w/d) in Voll- oder Teilzeit.

Ihr Aufgabengebiet umfasst u. a.

  • Erstellen von individuell angepassten Pflege und Betreuungsplänen unter Berücksichtigung gerontopsychiatrischer Fragestellungen in Zusammenarbeit mit der Pflege und Betreuung.
  • Erstellen bzw. Evaluieren von Konzepten und Prozessen mit gerontopsychiatrischem Inhalt (in Zusammenarbeit mit dem QM).
  • Beratung des Pflege- und Betreuungsteams und der Angehörigen
  • Schulung der Mitarbeiter
  • Überprüfung der Wirkung von Psychopharmaka und Mitteilung an Pflege und Ärzte
  • Mitarbeit bei Audits
  • Kontrolle der Pflegeplanungen in Bezug auf gerontopsychiatrische Fragestellungen (in Zusammenarbeit mit dem QM).

  • Fortbildung als gerontopsychiatrische Fachkraft
