Ihre Aufgaben:

  • Durchführung von Hausbesuchen im Rahmen der Hauskrankenpflege
  • Verantwortung für die unmittelbare und mittelbare Pflege vorwiegend älterer Menschen
  • Durchführung ärztlich übertragener Maßnahmen und Tätigkeiten im Rahmen medizinischer Diagnostik und Therapie
  • Medizinische Wundversorgung
  • Palliativpflege
  • Laufende pflegerische Evaluierungen der eingesetzten Therapieformen
  • Enge Vernetzung mit anderen Gesundheits- und Sozialberufen
  • Aufklärung der Betroffenen bzw. deren Angehörigen hinsichtlich Pflegebehelfe, Pflegegeld und weiterführender Unterstützungsleistungen
  • Dokumentation der durchgeführten Pflegemaßnahmen

Teilnahme an Teambesprechungen

Ihre Qualifikationen:

  • abgeschlossene Berufsausbildung zur/zum DGKP
  • Gute Deutschkenntnisse Bereitschaft zu regelmäßigen Abend- und Wochenenddiensten
  • Bereitschaft zu regelmäßigen Fort- und Weiterbildungen
  • Freude mit Umgang mit älteren Menschen
  • Selbstständiges Arbeiten und Flexibilität
  • Zuverlässigkeit, Einsatzbereitschaft und Teamgeist
  • Führerschein der Klasse B
