Description

Why work with us?

Our mission is to create access to high quality integrated health care and promote well-being for individuals, families, and our communities. At the heart of our organization are our people. Our team is passionate, dedicated, and supportive, creating a workplace where collaboration and service thrive to make a meaningful impact on our community

Job Duties:

As a Health Access Navigator, you help patients make sense of complex health insurance and coverage systems so they can get the care they need. You listen to questions and concerns, explain plan options in clear language, assist with applications and renewals, and problem-solve issues related to costs, billing, eligibility, and benefits. You also support patients with related barriers such as transportation, digital access, or paperwork literacy. Your work makes a direct difference in a patient’s ability to access medical, dental, behavioral health, and specialty care. If you enjoy learning systems, translating information, and helping people feel confident and supported, this role matters. Patients rely on you to reduce confusion, remove barriers, and ensure that care is possible-financially, logistically, and practically.
