Description
Under the direction of the ID Program Manager, the HIV Prevention Navigator provides comprehensive, patient-centered navigation services that support individuals who are eligible for or currently receiving HIV Pre-Exposure Prophylaxis (PrEP). The Navigator serves as a key member of the HIV prevention program within the Infectious Disease (ID) team, ensuring timely linkage to PrEP services, retention in care, medication access, adherence support, and ongoing patient engagement. This position works closely with medical providers, the pharmacy team, community health workers, and insurance payers to reduce barriers to HIV prevention services, and provides education and navigation for all PrEP modalities, including oral and long-acting injectable PrEP medications, while ensuring patients receive assistance with insurance coverage, prior authorizations, pharmaceutical assistance programs, and benefits coordination. The ideal candidate demonstrates cultural humility, strong organizational and communication skills, is persistent and resourceful, and shows a commitment to advancing health equity for populations disproportionately impacted by HIV. This is a grant-supported position funded through September 2027, with the possibility of extension contingent on availability of grant funds.
