**This is an in-office position with a Monday–Friday schedule from 8:00 a.m. to 4:30 p.m. The role also requires participation in an on-call rotation and flexibility to cover open site hours as needed**

This role operates administratively in a Home and Community-Based Services (HCBS) Setting, defined as “medical, social, and supportive services for Iowans with functional, cognitive, and other physical or mental health needs” (Iowa Health & Human Services). Rather than institutionalizing individuals who have these physical and mental health needs, HCBS settings provide these individuals with the opportunity to live and receive services in a way honoring to them and their abilities.

HCBS Coordinators provide leadership and support to a team that is laser-focused on delivering the best care possible. In this role, you’ll help fulfill the mission of empowering people to reach individual achievement across the spectrum of life. If you’re passionate about helping people reach their goals and strive for more, we want you to join our team. As a core team member, you’ll be responsible for monitoring and coordinating services, developing strategies to help individuals meet and exceed their goals, maintaining accurate and up-to-date records, and ensuring decisions and services are implemented in compliance with rules and regulations from governing bodies. You’ll emphasize individual choice and decision-making while collaborating with team members, individuals served, and support systems to create meaningful outcomes. Your leadership, organization, and dedication will provide the trusted foundation needed to make a positive difference in the lives of the people we serve.

What Winning Looks Like:

While it’s not a competition, we do recognize that each person wants to ‘win’ at life; and you’re a central part of someone else’s wins as well as your own! In this role, you’ll be responsible to:

  • Quality & Compliance Oversight: Ensure all services follow HCBS philosophy, agency policies, DHS regulations, and compliance standards; conduct site visits and maintain quality assurance across programs.
  • Individual Service Coordination: Develop, implement, and monitor Individual Service Plans with case managers and IDT; ensure assessments, goals, authorizations, and required documentation are current and accurate.
  • Medication, Finances & Daily Operations: Oversee medication administration processes, appointment follow-ups, and individual financial management including budgets, ledgers, and payee responsibilities.
  • Communication & Administrative Leadership: Serve as primary liaison with case managers, families, and stakeholders; manage incident reports, service documentation, transportation coordination, and general operational duties including reports, committees, and agency support tasks.
