We are seeking a Home Health Aide (CNA) to join our compassionate Hospice team. In this role, you will provide essential care and support to patients and families during end-of-life transitions, ensuring comfort and dignity every step of the way.

What You Do

• •Work 0.7 FTE. 56 hours bi-weekly
• •Provide care during 8-hour day shifts
• •Participate in on-call rotation every 4th weekend and 1 holiday per year

What You Need

• •High School Diploma or equivalent
• •Completion of a Home Health Aide/CNA program meeting state and federal requirements for placement on the WI & MN State Home Health Aide Registry upon hire
• •Minimum of 1 year CNA experience in acute care, nursing home, or home health agency
• •WI Registered Certified Nursing Assistant upon hire and MN Registered Certified Nursing Assistant within 90 days of hire
• •BLS CPR certification from American Heart Association or American Red Cross with hands-on learning
• •Valid driver’s license
