Sierra Health Care is looking for a Clinical Manager to lead home health clinical operations in Las Cruces. In this role, you'll oversee a team of nurses and therapists, guide individualized patient care planning, and make sure every patient receives safe, coordinated, and compassionate care. Key Responsibilities

• •Supervise and support nursing and clinical staff to ensure safe, patient-centered care delivery
• •Recruit, orient, and evaluate clinical staff, including ongoing competency validation
• •Oversee individualized patient care plans and lead care conferences
• •Coordinate closely with physicians, community partners, and the interdisciplinary team
• •Lead quality and compliance efforts through QAPI, infection prevention, and emergency preparedness
• •Ensure timely, accurate clinical documentation in the EMR
• •Stay available for consultation and on-call coverage to support continuity of care
• •Manage clinical staffing, scheduling, and resources to keep operations running smoothly

Requirements

What We're Looking For

• •Graduate of an accredited nursing or therapy program; current, unrestricted New Mexico license required
• •Minimum 3–5 years of clinical experience, including at least 1 year in home health or a related setting
• •Demonstrated ability to supervise clinical staff, coordinate care, and maintain regulatory compliance
• •Strong knowledge of Medicare and Medicaid Conditions of Participation and state licensure standards
• •Proven leadership, communication, and organizational skills
• •EMR proficiency and comfort managing multiple clinical priorities
• •Valid driver's license and reliable transportation
