We are hiring a Certified Nursing Assistant for a Home Health position providing care to patients in their homes in the Bridgeville, PA area.

What You’ll Do:

• •Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• •Provide hands-on care in the home, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support patient safety and independence
• •Educate and support patients and caregivers by reinforcing care plans, promoting healthy routines, and encouraging confidence with daily care tasks
• •Respond promptly to patient needs, communicate effectively with the care team, and maintain accurate, timely documentation in the EMR

• •Work independently, but never alone
• •Travel to patient homes within assigned territory

CNA Qualifications

• •Active Certified Nursing Assistant (CNA) certificate in the state you work
• •High School Diploma or certificate
• •Minimum one year healthcare experience
• •Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
