SCHEDULE- SAT & SUN 12HR SHIFTS & 1 DAY DURING THE WEEK OF YOUR CHOICE

• •Must be willing to travel within coverage area

As a Home Health RN with Elara Caring, you provide one-on-one care in the patient’s home. You manage your own caseload, build meaningful relationships, and see real progress in real time.

WHAT YOU WILL DO

• •Manage and coordinate your own home health caseload
• •Complete Start of Care, follow-up, and discharge visits
• •Perform comprehensive patient assessments
• •Develop and update individualized plans of care
• •Complete OASIS documentation accurately and timely
• •Educate patients and families on medications and disease management
• •Monitor patient progress and report changes to physicians
• •Collaborate with interdisciplinary care team members
• •Ensure documentation meets compliance and regulatory standards

WHAT YOU NEED

• •Active, unrestricted RN license
• •At least 1 year of nursing experience
• •Willingness to travel within the coverage area
• •Reliable vehicle, valid driver’s license, and auto insurance
• •Home Health experience preferred

Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.

Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
