We are hiring an After Hours Licensed Practical Nurse for a Hospice position providing end-of-life care to patients in their homes in the Springfield IL. and surrounding areas.

Work Schedule: Monday–Thursday, 4:00 PM–12:00 AM

What You’ll Do:

• •Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• •Provide after-hours support by conducting ongoing patient assessments focused on comfort, symptom management, and changes in condition, escalating clinical concerns to the RN as appropriate
• •Educate and support patients and caregivers during evenings, nights, and weekends by reinforcing end-of-life care plans, symptom relief strategies, and what to expect throughout the hospice journey
• •Ensure thorough, compliant EMR documentation

• •Work independently, but never alone
• •Travel to patient homes within assigned territory

After Hours LPN Qualifications

• •Active Licensed Practical Nurse (LPN) license in the state you work
• •Two years of LPN experience and the clinical expertise to deliver quality patient care, Hospice experience preferred
• •Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required
