As a Licensed Practical Nurse (LPN) , you’ll provide hands-on patient care under the guidance of a Registered Nurse, ensuring comfort, safety, and positive outcomes for patients in their homes. Key Responsibilities

• •Deliver compassionate, direct patient care in accordance with state regulations
• •Implement care plans developed by the RN and document progress accurately
• •Monitor patient conditions and communicate observations to the RN and physician
• •Collaborate with the care team to ensure seamless, high-quality service
• •Participate in professional development and team meetings

Requirements

• •Graduate of an accredited practical/vocational nursing program
• •Current LPN/LVN license in the State of Colorado
• •3 years of nursing experience preferred (Home Health or Med-Surg a plus)
• •Current CPR certification
• •Valid driver’s license, auto insurance, and reliable transportation

Pay Rate $26.00 - $ 33.00
