2pm-11pm afternoon shift

– $10,000 Sign-On Bonus

As a Hospice Registered Nurse (RN) , you will apply critical thinking and clinical expertise to perform comprehensive assessments, develop personalized care plans, and deliver high-quality, compassionate care. Your goal is to maximize the comfort, dignity, and well-being of patients and their families in alignment with the interdisciplinary plan of care and all regulatory standards. In the role of Hospice Admission RN , you will also be responsible for initiating the hospice plan of care and ensuring clear, timely communication in accordance with established guidelines.

What You’ll Do

  • Provide skilled nursing care in accordance with physician orders and organizational policies
  • Perform comprehensive assessments and develop individualized care plans
  • Collaborate with physicians, therapists, social workers, and home health aides
  • Supervise and support LPNs and Home Health Aides
  • Ensure compliance with regulatory standards and contribute to quality improvement initiatives
  • Promote patient satisfaction and positive health outcomes

What You Bring

  • Current RN license (BSN preferred)
  • Minimum 1 year of nursing experience ( required in California )
  • Home healthcare experience preferred
  • Valid driver’s license and reliable transportation
  • Strong computer skills and a commitment to ethical, mission-driven care

As a faith-based, not-for-profit agency, we serve patients and families in the comfort of home, offering skilled nursing, therapy (physical, occupational, speech), and medical social work. We are Medicare-certified and accredited by The Joint Commission .

Physical & Mental Requirements

  • Frequently lift, stand, and walk (35% of time)
  • Occasionally bend, stoop, twist, and assist with patient positioning (10%)
  • Independently lift up to 35 lbs
  • Perform detailed tasks with frequent interruptions and shifting priorities
  • Adapt to changing work environments and travel to various work locations
