Hospice Transition Nurse
Are you a compassionate nurse with a passion for guiding patients through the delicate transition from home health care to hospice?
Health Link is seeking a Transition Nurse to provide expert care and support to patients and their families during this important stage of care. If you are clinically skilled and have a heart for end-of-life care, we’d love for you to join our team.
JOB REQUIRES TO BE ON-SITE
Key Responsibilities:
Coordinate seamless transitions for patients moving from home health to hospice care.
Collaborate with home health clinicians to identify patients appropriate for hospice care, ensuring timely transitions.
Educate patients and families about hospice care, addressing concerns with empathy and understanding.
Provide on-site informational sessions to patients, families, and caregivers to explain the hospice transition process and available resources.
Educate home health care teams about hospice eligibility and ensure staff are well-equipped to identify when hospice is the right option.
Offer in-services and workshops to raise community awareness about hospice care and its benefits.
Collaborate with multidisciplinary teams, including physicians, therapists, and social workers, to support patients during the transition.
Perform comprehensive assessments of physical, emotional, and psychosocial needs during the transition process.
Serve as a primary point of contact for patients and families, offering support, resources, and counseling.
Maintain accurate and timely documentation of assessments and communications per healthcare regulations.
Qualifications:
Valid Nursing License in CA.
Minimum of 1 year of nursing experience, with preference for home health, palliative, or hospice care experience.
Strong communication and interpersonal skills to deliver compassionate care and education.
Ability to work both independently and as part of a multidisciplinary team.
In-depth knowledge of hospice and end-of-life care principles.
Pay Range:
RN : $55 to $70/hour
