What does a Family Tree Private Lead Community Care Provider do?

Provides personal assistance and health-related tasks per the Individualized Service Plan.
Maintains a clean, safe, and comfortable environment.
Practices infection control in accordance with agency policy.
Respects client privacy, dignity, and property.
Observes and reports changes in client condition to the office.
Schedules caregivers and ensures full shift coverage; monitors schedules daily and address open shifts.
Adds requested shifts and communicates changes to office and client/POA; provides updates to clients/POAs regarding scheduling changes.
Verifies completion of required daily tasks in WellSky and report gaps;
Ensures that care tasks are documented accurately and timely in WellSky
Trains and coaches’ new caregivers while escalating performance concerns to Market Director.
Serves as backup caregiver as needed to ensure continuity of care.
Meets with new clients to assess needs and establish schedules.
Conducts post-hospital visits to resume or adjust services.
Educates clients and families on available services.
Participates in the community’s scheduled activities to promote services.
Demonstrates effective time management and teamwork.
Attend training courses required to maintain competency and compliance with policies and local/state/federal regulations.
Supports agency operations and upholds policies and standards.
