Requirements

POSITION SUMMARY

Responsible for providing initial evaluations, individual, group, couples and/or family therapy. In addition, therapists will provide behavioral health consultation services to the Primary Care Teams within Pediatric and Adult outpatient clinics:

POSITION QUALIFICATIONS

• •Master's in Social Work
• •LCSW (California license)
• •California Driver’s License
• •Bilingual English/Spanish preferred

DUTIES AND RESPONSIBILITIES

• •Consultations about clients
• •Initial mental health evaluations resulting in realistic treatment plans
• •Assessing clients using standardized assessment/screening tools, measures, etc.
• •Provide brief, solution focused therapy;
• •Provide group psychoeducation/treatment related to chronic medical conditions;
• •Provide information and referral services;
• •Care coordination
• •Crisis assessments/triage with appropriate interventions
• •Provision of other services related to wellness
• •Documentation of services provided in accordance with requirements
• •Other related activities designed to provide integrated health care services
• •Individual psychotherapy with children, adults and families
• •Documentation and scheduling in electronic health record
• •Provide psychotherapy to children and adults; · other duties as assigned
