Location: Brenham, TX

  • Rate: $28-$31 per hr DOE

Job Summary

We are seeking compassionate and dependable LVN/LPNs to provide direct nursing care to residents at healthcare center. The LVN/LPN will support individuals with intellectual and developmental disabilities (IDD), psychiatric needs, and dual-diagnosis conditions , ensuring residents receive safe, quality, and person-centered care.

Key Responsibilities

Provide direct nursing care and support to residents in accordance with established care plans and nursing standards.

Monitor residents' health conditions and report changes or concerns to the appropriate healthcare professionals.

Administer medications and treatments as ordered and according to applicable policies and procedures.

Maintain accurate and timely nursing documentation and resident records.

Assist with assessing residents' health needs and responding appropriately to changes in condition.

Support individuals with IDD, psychiatric conditions, and dual-diagnosis needs in a respectful and compassionate manner.

Communicate effectively with residents, families, healthcare providers, and other members of the care team.

Follow facility policies, safety procedures, and applicable nursing regulations.

Work independently while also collaborating effectively with nurses, direct-care staff, and other healthcare professionals.

Adapt to changing resident needs and workplace situations.

Minimum Qualifications

Current, unencumbered LVN/LPN nursing license in Texas .

Experience working with individuals with IDD, psychiatric, and/or dual-diagnosis populations .

Must successfully pass a background check and drug screening .

Ability to work independently and as part of a team.

Ability to adapt quickly to changes and learn new procedures and responsibilities.

Strong communication, organization, and interpersonal skills.

Compassionate and patient-centered approach to resident care.

Preferred Qualifications

Previous experience in a residential, behavioral health, developmental disability, or long-term care setting.

Experience providing nursing care to individuals with complex behavioral or developmental needs.

Physical Requirements

Ability to perform the physical duties associated with direct patient/resident care.

Ability to stand, walk, bend, reach, and assist residents as required.

Ability to respond promptly to resident needs and emergency situations.

Additional physical requirements may apply based on facility and resident-care needs.
