As a LPN/LVN Clinical Coordinator, you’ll contribute to our success in the following ways:

• •Reviews and clears HCHB coordination notes and processes administrative task workflows in an accurate and timely manner.
• •Obtains orders from physicians, including verbal, written and faxed orders, and enters into appropriate system for CTM review and approval.
• •Monitors patient hospitalization list, confirms status, and coordinates appropriate resumption of care.
• •Discharges patients in inpatient facility at end of episode (EOE)
• •Obtains patient-related attachment renewal.
• •Participate in High-Risk Meetings and ensure that the best provision of services is offered.
• •Available to provide direct care, as needed.
• •Answers telephone calls from patients, patient representatives, and medical professionals and communicate messages to the appropriate party or parties.
• •Provides clerical support to patients such as but not limited to scheduling follow-up appointments for patients (as requested), providing health summaries to case managers, obtaining pre-authorizations, and addressing patient questions or concerns.

What is Required?

• •Graduate of an accredited school of professional nursing
• •Current Certified Licensed Vocational Nurse or Licensed Practical Nurse
• •1 year of home care or hospice experience
• •Able to seek patient feedback, advocate for patients, understanding of patient needs, and managing patient expectations
• •Able and willing to travel up to 50% as needed to support business and patient needs
• •Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• •Valid driver’s license and insurance and reliable transportation to perform job tasks
