Description

Are you called to serve others through compassionate nursing care? Sisters of Notre Dame de Namur is a long-term care residence for Sisters who have dedicated their lives to serving God. We are dedicated to providing compassionate, high-quality care to our Sisters in a faith-based environment.

We are seeking a detail-oriented and compassionate (Minimum Data Set) MDS Coordinator to join our healthcare team in our 80-bed, five-star facility. This vital role coordinates MDS assessments, care planning, clinical documentation, and regulatory compliance while supporting our interdisciplinary team. The MDS Coordinator plays an important role in ensuring accurate assessments, quality resident care, and appropriate reimbursement. This is more than a job; it is an opportunity to make a meaningful difference in the lives of our Sisters.

This is a full-time, day shift position. Occasional scheduling flexibility may be required to meet assessment deadlines, attend meetings, or support clinical needs.

We welcome licensed nursing professionals to apply, with a strong preference and priority given to Registered Nurses (RNs). Licensed Practical Nurses (LPNs) are welcome to apply; however, this position requires additional RN oversight for documentation and clinical review in accordance with regulatory requirements.

Requirements

Key Responsibilities:

  • Lead and coordinate completion of all MDS assessments in accordance with the Resident Assessment Instrument (RAI) Manual.
  • Maintain and communicate MDS assessment and Care Plan schedules, including due dates, to the interdisciplinary team (IDT).
  • Ensure accurate and timely completion of MDS, Care Area Assessment (CAA), and Care Plan documentation, including required electronic submissions to CMS.
  • Participate in Care Conferences, IDT meetings, Risk Management meetings, and other required committees.
  • Monitor resident census and clinical status, adjusting assessment schedules as needed.
  • Coordinate skilled services, including coverage decisions, assessment timing, and scheduling, to support appropriate reimbursement.
  • Communicate skilled census information and documentation requirements to internal and external partners.
  • Support reimbursement accuracy through timely assessments, interdisciplinary planning, and staff education.
  • Oversee the RAI process and ensure compliance with applicable federal and state regulations.
  • Schedule and coordinate required nursing and IDT assessments, ensuring documentation is completed in the electronic health record.
  • Provide backup support for Care Plan Nurse responsibilities as needed.
  • Review care and documentation for residents receiving skilled services and follow up with nursing staff as indicated.
  • Communicate changes in skilled status and hospice enrollment to the Finance Department and IDT.
  • Complete ICD-10 coding of medical diagnoses and collaborate with physicians, Medical Records, and Therapy to support accurate diagnosis coding.
  • Monitor Medicaid resident status and coordinate services and documentation to support accurate case-mix classification and reimbursement.
  • Maintain current knowledge of federal and state MDS requirements.
  • Assist with survey preparation, including completion of required census and resident roster documentation.
  • Ensure appropriate backup coverage for assigned MDS responsibilities.
  • Attend required employee and departmental meetings and in-service training.

Skills and Qualifications:

  • Active, valid nursing license in the State of Ohio; RN strongly preferred.
  • Minimum of 1–2 years of healthcare experience, preferably working directly with older adults in geriatrics or long-term care.
  • Experience with the MDS/RAI assessment process and comprehensive care plan development required.
  • Knowledge of Medicare and Medicaid regulations and long-term care documentation requirements.
  • Experience with electronic health records; familiarity with PointClickCare preferred.
  • Strong organizational, analytical, communication, and problem-solving skills.
  • Ability to work independently, exercise sound judgment, prioritize responsibilities, and meet deadlines.
  • Ability to recognize problems and seek guidance or escalate concerns when appropriate.
  • Professionalism, integrity, and commitment to confidentiality and facility policies.
  • Knowledge of the needs and limitations of older adults.
  • Commitment to serving our Sisters with kindness, respect, and compassion.
