Position Overview

PharmD Live is seeking a detail-oriented, patient-focused Medical Assistant to support our telehealth care management program. This role combines patient outreach, appointment scheduling, care coordination, and core Medical Assistant support in a remote, high-volume environment.

Key Responsibilities

Conduct outbound and inbound patient calls and schedule appointments

Support Annual Wellness Visits and care management outreach

Perform medication reconciliation and update patient information

Document patient interactions accurately in the EHR

Prepare charts and support care coordination and follow-up

Maintain HIPAA compliance and escalate clinical concerns appropriately

Meet established outreach, scheduling, and documentation targets

Qualifications

Medical Assistant certification or equivalent healthcare experience required

Prior healthcare, patient outreach, scheduling, or care coordination experience

Strong communication, organization, and documentation skills

Comfortable with high-volume calling and performance metrics

EHR experience preferred

Medicare patient experience preferred

Reliable and self-motivated in a remote environment
