What You’ll Do

• ✔ Assess patients’ psychosocial, financial, and environmental needs
• ✔ Complete initial and ongoing social work evaluations
• ✔ Support care planning and discharge planning
• ✔ Connect patients and families with community resources
• ✔ Communicate changes and concerns with care team and physicians
• ✔ Document visits and maintain compliance with care standards
• ✔ Provide counseling and guidance to patients and caregivers

What You Need

• ✔ Master’s or Doctoral Degree in Social Work (CSWE accredited)
• ✔ Active Social Worker license
• ✔ 1+ year of healthcare social work experience
• ✔ Strong communication and interpersonal skills
• ✔ Ability to travel within the coverage area (50%)
• ✔ Valid driver’s license, auto insurance, and reliable transportation
• ✔ Ability to lift, push, or pull up to 50–100 lbs
