What you’ll do as a Medical Social Worker (MSW) Case Manager: Conducting comprehensive psychosocial assessments of patients and their families to identify their needs, strengths, and resources

• •Developing and implementing care plans that address the psychosocial needs of patients and their families
• •Providing counseling and emotional support to patients and their families, including grief and loss counseling
• •Coordinating community resources and services to meet the needs of patients and their families
• •Serving as an advocate for patients and their families, helping them to navigate the healthcare system and access needed resources
• •Participating in interdisciplinary team meetings to develop and review patient care plans
• •Maintaining accurate and timely documentation of all services provided

Requirements for Medical Social Worker (MSW) Case Manager:

• •Master's degree in Social Work from an accredited school of social work
• •Current state licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW)
• •Minimum of 2 years of experience working in hospice or palliative care
• •Excellent communication and interpersonal skills
• •Ability to work effectively as part of an interdisciplinary team
• •Knowledge of community resources and services that can support the needs of hospice patients and their families
• •Ability to provide compassionate and empathetic care to patients and their families
