Nurse Acenda Integrated Health is recruiting for a Nurse Home Visitor to support the Nurse Family Partnership in Atlantic County, New Jersey.

Key Responsibilities:

• •Travel to clients' homes and provide home visits to first-time mothers providing education and support through pregnancy and up until the child's 2nd birthday.
• •Work to improve pregnancy outcomes, child health and development outcomes, and the economic self-sufficiency of families.
• •Build long-term therapeutic relationships with clients and their families.
• •Provide community resources as needed per nursing assessment.
• •Adheres to the nursing process and the Nurse-Family Partnership model of home visitation.
• •Attends and participates in case conferences and team meetings.
• •Collaborates with client and family to develop short- and long-term goals.
• •Formulates nursing diagnosis based on nursing assessment, client goals and Nurse-Family Partnership outcomes.

Requirements:

• •Bachelors degree in Nursing (BSN) required
• •Active NJ Registered Nurse license (in good standing)
• •Must have and maintain a valid driver's license, use of an insured vehicle and an acceptable driving record
• •Professionalism and a commitment to excellence in care
