Full Time/First Shift

The Nurse Navigator serves as the primary point of contact for physicians and other healthcare providers, patients and their caregiver/family, and other resource-based entities, providing and accessing clinical and supportive care services offered within the multidisciplinary teams at Baptist Health Medical Group and other entities as needed. The Nurse Navigator serves as an integral part of the care team within the Baptist Health Medical Group coordinating effective and comprehensive services through the continuum of care for patients with treatment and support service options.

This position requires a collegial and collaborative working style to fulfill these specific work functions.

Minimum Education, Experience, Training and Licensures Required :

• •Graduate of an accredited School of Nursing with current RN licensure by the Commonwealth of Kentucky State Board of Nursing required.
• •Bachelor of Science in Nursing required, or higher.
