Key Responsibilities

• •Develop and implement individualized care plans
• •Provide therapy services per physician’s orders
• •Assess functional status and recommend adaptive equipment
• •Teach new skills or retrain patients in daily activities
• •Maintain accurate documentation and participate in case conferences

Requirements

• •Licensed Occupational Therapist (OT) in the State of Colorado
• •Graduate of an ACOTE-accredited OT program, eligible for NBCOT certification
• •Minimum 2 years’ clinical experience (home health preferred)
• •Current CPR certification
• •Valid driver’s license, insurance, and reliable transportation
