Hometown Home Health is a full service home health care agency. We offer a wide range of skilled medical home care services and clinical nursing care in the Greater Jacksonville and St. Augustine areas.

What We’re Searching For

Our team is searching for a licensed Occupational Therapy Assistant in the state of Florida who prioritizes patient care.

What You’ll Do

• •Provide occupational therapy services and assessments to patients in their homes
• •Guide and instruct patients through prescribed therapeutic care.
• •Assist in the implementation and development of patient care plans.
• •Prepare progress notes following visits for patients’ charts.

What You Need:

• •Graduated after successful completion of an Occupational Therapy Assistant education program accredited by:
• •The Accreditation Council for Occupational Therapy Education (ACOTE) of the American Occupational Therapy Association, Inc. (AOTA), or its successor organizations.
• •Is eligible to take, or successfully completed, the entry-level certification examination for Occupational Therapy Assistants developed, and administered, by the National Board for Certification in Occupational Therapy, Inc. (NBCOT)
• •Licensed to practice within the state of Florida
• •Member of the American Occupational Therapy Association preferred.
• •CPR certification required (Online certification not accepted).
• •At least one year of institutional experience
• •Reliable Transportation
