Mit der Assistenz beim Wohnen unterstützen wir Menschen mit geistiger Beeinträchtigung bei einem selbstständigen Leben im eigenen Wohnraum, indem wir sie anleiten und beraten.

Wir arbeiten nach dem Grundsatz der individualpädagogischen, personenzentrierten Begleitung. Im Zentrum stehen für uns die individuellen Bedarfe und die Selbstbestimmung der Kund\*innen.

Die Stunden (bis zu 35 Wochenstunden) können anteilig vergeben werden.

### Aufgaben

  • Beratung, Assistenz und Begleitung von Menschen mit Beeinträchtigungen im lebenspraktischen und sozialen Bereich (z.B. Suche und Erhalt von Arbeit und Wohnung, Haushaltsführung, Arztbesuche oder Freizeitgestaltung)
  • bei Bedarf Übernahme pflegerischer und medizinisch erforderlicher Aufgaben
  • Planung, Durchführung und Dokumentation von Assistenzleistungen
  • Zusammenarbeit mit Ämtern, gesetzlichen Betreuer\*innen, Angehörigen, Fachärzt\*innen und Psycholog\*innen
  • Teilnahme an Fallbesprechungen und Fachberatung

### Profil

  • abgeschlossene Ausbildung zum Heilerziehungspfleger (m/w/d) oder vergleichbar
  • Erfahrung in der Begleitung von Menschen mit Beeinträchtigung wünschenswert
  • Zuverlässigkeit
  • selbstständige, strukturierte Arbeitsweise
  • Führerschein der Klasse B und Bereitschaft zur Nutzung des privaten PKW
  • Bereitschaft, vorwiegend nachmittags zu arbeiten
