About This Role Join Alive, Middle Tennessee's original hospice and a trusted leader in end-of-life care for 50 years, as a Palliative Care APP embedded in our community hospitals. You'll serve as an inpatient palliative care consultant, helping patients and families navigate complex illness through expert symptom management, goals-of-care conversations, and advance care planning. This is an opportunity to build meaningful relationships with hospital teams, grow your own referral base, and practice at the top of your license with the backing of a respected, mission-driven organization. Hospitals include Centennial, Ascension St. Thomas West and Midtown, Summit, Hendersonville Medical Center, and Skyline.

What You'll Do

  • Provide palliative care consultations alongside hospital medical teams, managing symptoms related to acute and chronic conditions
  • Lead compassionate conversations with patients and families about goals of care, prognosis, and treatment options
  • Develop and implement individualized care plans in collaboration with physicians, social workers, chaplains, and case managers
  • Build and nurture referral relationships within your assigned hospital
  • Order and interpret labs, imaging, and medications; document thoroughly in the medical record
  • Serve as a resource and mentor to students, residents, and new team members

Why Alive

  • Mission that matters: For 50 years, Alive has helped Middle Tennesseans live fully and die with dignity. You'll be part of that legacy.
  • Autonomy with support: Practice independently while backed by experienced physicians and a collaborative interdisciplinary team.
  • One campus: This role is assigned to Ascension Saint Thomas Midtown. Opportunities to float may arise.
  • Room to grow: Opportunities for leadership, mentorship, and specialty certification support.

Compensation & Benefits

  • Comprehensive medical, dental, and vision insurance
  • Generous PTO
  • 403(b) retirement plan with employer match
  • CME allowance and certification support
  • Strong work life balance

Requirements

What You Bring

  • Graduate of an accredited NP, CNS, or PA program
  • Active, unrestricted Tennessee license and national certification
  • Unrestricted DEA
  • Valid driver's license and insurance

Preferred:

  • Experience in hospice, palliative care, ICU, or inpatient acute care
  • ACHPN or HPCC certification (or willingness to pursue)

We welcome candidates with strong acute care backgrounds who are ready to transition into palliative medicine. We'll support your growth.
