Palliative Fachkraft im ambulanten Pflegedienst

## Ihre Aufgaben

  • Fachgerechte palliative Pflege und Betreuung von Patient:innen im häuslichen Umfeld
  • Symptomkontrolle (z. B. Schmerz-, Atemnot- und Angstlinderung) in Zusammenarbeit mit Ärzt:innen
  • Individuelle Pflegeplanung und -dokumentation unter Berücksichtigung palliativer Standards
  • Beratung und Begleitung von Patient:innen und Angehörigen in Krisensituationen
  • Kooperation mit Haus- und Fachärzt:innen, Hospizdiensten, SAPV-Teams und weiteren Netzwerkpartnern
  • Anleitung und Unterstützung von Kolleg:innen im Team bei palliativen Fragestellungen
  • Teilnahme an Fallbesprechungen und interdisziplinären Teambesprechungen

## Ihr Profil

  • Abgeschlossene Ausbildung als examinierte Pflegefachkraft (Gesundheits- und Krankenpfleger:in oder Altenpfleger:in)
  • Zusatzqualifikation in Palliative Care oder die Bereitschaft, diese zu absolvieren
  • Erfahrung in der ambulanten Pflege oder palliativen Versorgung wünschenswert
  • Hohes Maß an Empathie, Einfühlungsvermögen und Kommunikationsstärke
  • Selbstständige und verantwortungsbewusste Arbeitsweise
  • Führerschein Klasse B
