### Deine Aufgaben

Du begleitest schwerstkranke Menschen medizinisch und menschlich in ihrem Zuhause. Gemeinsam mit unserem multiprofessionellen Team sorgst Du dafür, dass Lebensqualität, Selbstbestimmung und Symptomkontrolle jederzeit im Mittelpunkt stehen.

Zu Deinen Aufgaben gehören:

  • Palliativmedizinische Betreuung unserer Patientinnen und Patienten
  • Enge Zusammenarbeit mit Hausärzten, Fachärzten, Kliniken und Pflegeeinrichtungen
  • Beratung und Begleitung von Patienten und Angehörigen
  • Teilnahme an unserem vergüteten Rufbereitschaftssystem
  • Mitwirkung bei Fallbesprechungen, Supervisionen, Qualitätszirkeln und Fortbildungen
  • Aktive Mitgestaltung der Weiterentwicklung unseres SAPV-Teams
