Pflege

  • Beratung zu Pflegegraden und Leistungen der Pflegeversicherung (§37.3 SGB XI)
  • Organisation ambulanter Pflege und Betreuung zu Hause
  • Unterstützung von Patienten bei Anträgen und Burokratie rund um die häusliche Pflege
  • Enge Abstimmung mit Angehörigen und Hausärzten / stationären Einrichtungen / Wundmanager / ...

Personalführung

  • Fort- und Weiterbildungsplanung
  • Interne Fortbildungen: Expertenstandards, Datenschutz, Hygiene, Sicherheit.
  • Anleitung und Unterstützung des Pflegeteams
  • Einarbeitung neuer Mitarbeitender

Qualitätssicherung

  • Pflegevisite von Patienten und Dokumentation
  • Umsetzung von Pflegestandards und gesetzlichen Vorgaben
  • Vorbereitung von MD-Prüfungen oder Qualitätsprüfungen
  • Weiterentwicklung von Pflegekonzepten

Kommunikation & Koordination

  • Zusammenarbeit mit Ärzten, Therapeuten und Angehörigen
  • Beratung von Patienten und Angehörigen
