Die Altenhilfe-Zentrum St. Clemens Münster-Hiltrup gGmbH ist in der Unterstützung älterer und pflegebedürftiger Menschen in Münster-Hiltrup aktiv. Wir betreiben zwei Tagespflegen mit je 15 Gästen, eine stationäre Einrichtung für 83 Bewohnern in 5 Wohngruppen sowie eine gerontopsychiatrische Einrichtung mit 42 Bewohnern in 3 Hausgemeinschaften.

Ihre Aufgaben:

Bewohnerorientierte, aktivierende und betreuende Grund- und Behandlungspflege

-

Verantwortung im Rahmen der Bezugspflege

-

Pflegeplanung und -dokumentation nach dem Strukturmodell

-

Umfassende Angehörigenarbeit und Arztkommunikation

-

Einhaltung von Maßnahmen zur Qualitätssicherung

Sie bringen mit:
