## Ihre Aufgaben

  • Patientenversorgung in der letzten Lebensphase
  • Mitwirkung bei der Umsetzung individueller Pflegepläne
  • Dokumentation der durchgeführten Pflegemaßnahmen
  • Kooperation mit Pflegefachkräften und interdisziplinären Teammitgliedern

## Ihr Profil

  • Abgeschlossene Ausbildung als Pflegefachkraft (m/w/d) o.ä.
  • Empathie und respektvoller Umgang mit Pflegebedürftigen
  • Teamfähigkeit und zuverlässige Arbeitsweise
  • Bereitschaft zu flexiblen Arbeitszeiten im ambulanten Pflegedienst
  • Weiterbildung in Palliativ Care
  • Führerscheinklasse B
