Pflegefachkraft für die Phase F (m/w/d)

Diese Tätigkeiten erwarten Sie

Sicherstellung der bestmöglichen Pflege und Betreuung nach den Vorgaben
des SGB XI und V
Umsetzung des Konzeptes zur Palliativversorgung
Durchführung von Fortbildungen der Pflege- und Betreuungskräfte
Ansprechpartner für Hospize und Palliativteams

Das zeichnet Sie aus

Abgeschlossene Berufsausbildung als examinierte Pflegefachkraft (Altenpflege und / oder Gesundheits- und Krankenpflege)
Erfolgreich abgeschlossene Weiterbildung in Palliative Care
Sie arbeiten eigenständig und lösungsorientiert
Sie verfügen über Kenntnisse in der digitalen Pflegedokumentation
