  • ~ Abgeschlossene Ausbildung als Gesundheits- und Krankenpfleger, Gesundheits- und Kinderkrankenpfleger, Altenpfleger oder Pflegefachkraft
  • ~ Idealerweise Weiterbildung und Erfahrung im Bereich Pflegeberatung
  • ~ Gute Kenntnisse der gesetzlichen Grundlagen, insbesondere SGB XI
  • ~ Kommunikationsstärke und Einfühlungsvermögen im Umgang mit Menschen
  • ~ Selbständige und strukturierte Arbeitsweise
  • ~ Führerschein Klasse B
  • Deine Aufgaben
  • ~ Aufbau des zugewiesenen Gebietes
  • ~ Durchführung von Beratungsbesuchen nach § 37 Abs. 3 SGB XI
  • ~ Individuelle Beratung und Schulungen von Pflegebedürftigen und deren Angehörigen nach § 45 SGB XI
  • ~ Unterstützung bei der Organisation und Sicherstellung der häuslichen Pflege
  • ~ Einschätzung von Pflegesituationen und Erarbeitung passender Versorgungspläne
  • ~ Dokumentation der Beratungseinsätze gemäß gesetzlichen Vorgaben
  • ~ Zusammenarbeit mit Pflegekassen, Ärzten und weiteren Netzwerkpartnern
  • ~ Unterstützung bei der Weiterentwicklung unserer Beratungsstrukturen
