Die DIAKONIE Soziale Dienste gGmbH ist eine Tochtergesellschaft des Diakonieverein e.V. Bitterfeld-Wolfen-Gräfenhainichen und in der ambulanten und teilstationären Altenhilfe tätig.

Zur Erweiterung unserer Personalkapazitäten und zum nachhaltigen Ausbau unserer ambulanten Dienstleistungsangebote suchen wir ab sofort eine engagierte

Pflegefachkraft (m/w/d)

für die ambulante Pflege.

## Ihre Aufgaben

  • Mobile Betreuung und Versorgung unserer Patientinnen und Patienten
  • Medizinische und seelische Betreuung unserer Kundinnen und Kunden
  • Beratung und Anleitung von Pflegebedürftigen und deren Angehörigen
  • Unterstützung bei der täglichen Körperpflege und Versorgung
  • Durchführung der Krankenbeobachtung
  • Fachgerechte Pflegedokumentation
  • Durchführung und Evaluation vorbeugender und spezieller Pflegemaßnahmen
  • Individuelle und wertschätzende Begleitung unserer Kundinnen und Kunden im häuslichen Umfeld

## Ihr Profil

Sie verfügen über:

  • Eine abgeschlossene Ausbildung als Altenpfleger/in oder Gesundheits- und Krankenpfleger/in
  • Verantwortungsbewusstsein und Zuverlässigkeit
  • Teamgeist und soziale Kompetenz
  • Einfühlungsvermögen und Freude am Umgang mit hilfsbedürftigen Menschen
  • Selbstständige und sorgfältige Arbeitsweise
  • Führerschein der Klasse B
