  • Koordination der Palliativversorgung, Aufnahmeplanung und Sicherstellung der Versorgung inklusive der Dokumentation der SAPV und der Brückenpflege
  • Vernetzung und Kooperation mit Ärzten und anderen Diensten/ Einrichtungen des Gesundheits- und Sozialwesens
  • Personaleinsatzplanung/ Dienstplangestaltung
  • Mitarbeit in der Öffentlichkeitsarbeit
  • Qualitätsentwicklung und Evaluation der SAPV und Brückenpflege
  • Planung und Organisation von Fortbildungen, Supervisionen und Qualitätszirkeln und Gremienarbeit
  • Pflegefachkraft mit abgeschlossene Zusatzausbildung Palliative Care (160 Stunden Kurs)
  • Mindestens dreijährige Berufserfahrung in der Palliativ- und/oder Hospizarbeit sowie im ambulanten Bereich
  • von Vorteil ist eine mehrjährige Leitungserfahrung und ein Studium /Weiterbildung im Pflegemanagement
  • Hohe soziale, fachliche und kommunikative Kompetenzen
  • Verständnis für die Belange palliativer Patienten und ihrer Angehörigen
  • Organisatorisches Geschick und Teamfähigkeit
