Job Details:

  • The Medication History Pharmacy Technician, with supervision of a pharmacist, assists in the process of obtaining
  • medication history for patients who will be admitted to the hospital. The focus of this technician is the accurate
  • collection of prescription information, allergy information, patient and/or family/guardian interview, and
  • documentation of those activities. The complete collection of medication information may also include contacting
  • pharmacies, long-term care facilities, and prescribers.
  • Additionally, the Medication History Pharmacy Technician works under the supervision of the pharmacist following
  • established policy and procedures set forth by the hospital and pharmacy department. This role meets the
  • requirements of being a qualified pharmacy technician as established by the State Board of Pharmacy and holds a
  • Certified Pharmacy Technician license from a State Board of Pharmacy accredited agency. The technician adheres to
  • safety practices to ensure quality care for patients and maintains competencies outlined in pharmacy department
  • policies. The technician shows familiarity with pharmacy practice settings, legal requirements and limitations,
  • prescription information and calculations, drug identification and generic equivalents, drug manufacturing,
  • packaging and labeling information, and proper drug handling and storage practices. Department Specific Job Details:

Hours: 8-days on/6-days off- evening shift-

Education:

HSD/GED is required

Certification/Licensure:

CPhT - Certified Pharmacy Technician - State of Ohio Board of Pharmacy is required
