Requirements

About the Role

As a Physical Therapist Assistant (PTA), you'll provide skilled physical therapy treatments under the direction and supervision of a licensed Physical Therapist in a Skilled Nursing Facility (SNF). You'll work with patients recovering from surgery, illness, injury, and hospitalization while helping them safely improve mobility, balance, endurance, strength, and functional independence.

Responsibilities

• •Deliver physical therapy treatments established by the supervising Physical Therapist.
• •Monitor patient response to treatment and communicate progress with the Physical Therapist.
• •Assist patients in improving mobility, balance, gait, strength, endurance, and functional independence.
• •Accurately document treatments, patient progress, and daily billing.
• •Collaborate with interdisciplinary team members to provide coordinated patient care.
• •Educate patients, families, and caregivers on home exercise and maintenance programs.
• •Participate in care planning and discharge planning as appropriate.
• •Maintain compliance with all state licensure requirements and Concept Rehab policies.

Qualifications

• •Associate Degree from an accredited Physical Therapist Assistant program.
• •Current Physical Therapist Assistant (PTA) license in the state of practice (or eligible for licensure).
• •Strong communication and teamwork skills.
• •Passion for helping older adults maximize mobility, safety, and independence.
