Requirements

What You'll Do

Under the supervision of a Licensed Physical Therapist, you will:

• •Deliver physical therapy treatments according to established plans of care
• •Help patients improve mobility, strength, balance, function, and independence
• •Communicate patient progress and concerns with the supervising Physical Therapist and interdisciplinary care team
• •Collaborate with physicians, nursing staff, caregivers, and families to support patient outcomes
• •Participate in care planning and interdisciplinary team meetings as appropriate
• •Provide education and training to patients, families, and facility staff
• •Complete timely electronic documentation, treatment notes, and daily billing activities
• •Assist with discharge planning and continuity of care
• •Support restorative and functional maintenance programs within the facility
• •Maintain productivity and documentation standards while delivering high-quality patient-centered care

Qualifications

• •Graduate of an accredited Physical Therapist Assistant program
• •Associate degree as a Physical Therapist Assistant
• •Current PTA license in the state of practice
• •New graduates are encouraged to apply
• •Strong communication, teamwork, and clinical problem-solving skills
• •Passion for delivering exceptional patient-centered care in a Skilled Nursing Facility setting
